CRESTVIEW, FL – An 81 year-old woman, driving a Lincoln passenger car, was killed in a fatal semi truck accident in Crestview, FL on July 31. The city of Crestview is located in Okaloosa County, Florida in the panhandle area of the state about 52.5 miles northeast of Pensacola.

The fatal accident happened just before 9:00 a.m. Wednesday morning, July 31, at the intersection of Florida State 85 and Phil Tyner Road. The victim, identified as 81 year-old Doris Hart from Crestview, was pulling into traffic onto Florida State 85 from Phil Tyner Road, when her car collided with a semi truck traveling south on Highway 85. Her car was struck on the driver’s side by the 18 wheeler.

The semi truck driver was identified as Desi Arnaz Kirkland Tate, from Rockledge, FL. The city of Rocledge is located more than 450 miles away on the Eastern seaboard. Tate did not appear to be injured in the incident.

The 18-wheeler accident is being investigated by the Florida Highway Patrol who believe that Hart failed to yield the right of way to the big rig.
